For vaccinated guests, please submit proof of vaccination for you and all your guests by July 25th. You may use the form below or you can email a photo of the card(s) to Meredyth. Please include your guest names with each card. Disregard if you have already submitted your record.
You can return to this page to upload additional cards at a later date but all submissions must be in by Monday July 25th.
